Claire Wineland, 21, is famous for her story of illness and death on social media.
Ms. Wineland suffered a double lung transplant on August 26, but suffered a stroke shortly afterwards.
His foundation said he had died safely, surrounded by families.
In her posting on her Facebook page, Claire's Place Foundation said: "Our inspirational founder died. He was not in pain and the medical staff said it was the most peaceful part they had ever seen."
At TEDx talks in 2017, Ms. Wineland spoke about the importance of self-esteem when living with a disease that shortens life.
"Life isn't just about being happy ... It's about what you do in your life and whether you can find deep pride in who you are and what you give," he said.
Ms. Wineland was radical Senator Bernie Sanders, and he fled to the hospital to attend one of his meetings.
The 2016 Democratic presidential candidate honors him at his post on Twitter.
In his last post on social media before being transferred, Ms. Wineland spoke about the importance of organ donation for those suffering from cyst fibrosis.
"I thank the doctors who will tempt these lungs and give me more life to work with me. I am grateful for the opportunity to continue to be one," he said.
Organs are provided for transplantation.
"His right kidney was transferred to a 44-year-old woman in San Diego, and his kidney was transplanted to a 55-year-old man in northern California," his mother, Melissa Yeager, told CNN.
Cystic fibrosis is a known genetic condition without healing. This progressive disease causes too much mucus in the lungs and other organs, causing infection and digestive problems.
The US-based Cystic Fibrosis Foundation says more than 70,000 people live around the world with an average age of 40 years.
Claire Wineland became famous through positive but realistic reports about living with cyst fibrosis.
